Successful resection of an undifferentiated sarcoma in a child using a real-time surgical navigation system in an open magnetic resonance imaging operation room

Case report

A 17-month-old girl presented at another hospital with symptoms of swelling of the right thigh and right inguinal lymphadenopathy. An abdominal CT scan showed a 5-cm calcified mass located in the right side of the pelvis (Fig. 1) and right inguinal lymphadenopathy. Discussion

The open MRI navigation system was effective in this case, suggesting that it may be applicable in clinical pediatric surgery particularly for pediatric malignancy. It uses a single and quick registration method and produces an acceptable range of registration error in clinical surgery.

    Advancement of imaging technology might enable detection of minute lesions and the lesions may be invisible with the naked eye during surgery. Adaptation of an augmented reality navigation system was reported to be useful [2,3], but it is still in the research stage. Therefore, our method, which was previously used to mark deep impalpable pulmonary nodules [4] and was applied to a tiny abdominal lesion, is easy to perform and can be used in clinical practice.
